Heroin addiction treatment in Malaysia: residential opioid rehab
Heroin addiction treatment in Malaysia starts with a medical conversation, because stopping an opioid is very uncomfortable and the weeks afterwards carry real risks. Salus offers opioid addiction treatment as a 90-day residential stay in Cyberjaya, Selangor, with 24-hour medical oversight and therapy that carries on long after withdrawal has passed. If the person already takes methadone or buprenorphine, tell us when you call.
Signs that heroin or opioid use has become dependence
Some of these show up when the person is using and some when they are not. Seeing one or two is reason enough to ask for advice.
- Sudden drowsiness, nodding off mid-sentence, or slow and slurred speech.
- Sweating, yawning, a runny nose, aches and restlessness that ease once the person has used again.
- Needing more of the drug for the same effect, or taking more of a painkiller than was prescribed.
- Money, jewellery or electronics going missing, and debts that nobody can explain.
- Syringes, burnt foil, straws or small bags hidden in the house, the car or the workplace.
- Days of disappearing, missed shifts, and promises to stop that keep being broken.
How heroin addiction treatment works at Salus
It begins with a medical review and a psychiatric assessment on arrival, so the team knows what the person has been using, how much, and which medicines they already take. The medical team manages withdrawal safely and comfortably, with care round the clock. Therapy and planning for life afterwards start as the body settles, because withdrawal is the beginning of treatment and only a small part of it.
01
Medical review first
The team checks everything the person has been using, including alcohol, sedatives or cough syrup alongside heroin. Some people need hospital care before a residential stay, and we will say so honestly if that applies.
02
Withdrawal managed
Symptoms start within about 12 hours of the last heroin. They are very uncomfortable but not life threatening in themselves, and the team keeps the person safe and comfortable throughout.
03
A conversation about medication
The medical team asks what medication the person already takes and reviews it as part of the assessment. Nobody should stop a prescribed medicine on their own before coming, and no medicine is promised in advance.
04
Therapy for what sits underneath
The diagnosis decides the therapy: cognitive behavioural therapy, motivational interviewing, REBT, solution-focused or trauma-focused work. Depression, anxiety, trauma and poor sleep are treated in the same plan.
05
Rebuilding daily life
A Salus Clinician works alongside medical care to plan recovery across six areas of life, through four phases that move on when the person is ready. Family sessions run throughout, and a written aftercare plan is ready before leaving.

What is heroin withdrawal like, and is it safe to do at home?
Symptoms usually start within 12 hours of the last heroin. Early on come agitation, anxiety, muscle aches, a streaming nose and eyes, sweating, yawning and poor sleep. Later come stomach cramps, nausea, vomiting and diarrhoea. MedlinePlus describes this as very uncomfortable but not life threatening. The discomfort is intense and relief is always one dose away, which makes stopping alone at home hard to sustain.
The greater danger comes after withdrawal. Tolerance falls quickly once someone stops, so a dose that used to be normal can now cause an overdose. MedlinePlus notes that most opioid overdose deaths happen in people who have just detoxed, and the World Health Organization (WHO) lists resuming use after detoxification as a risk factor. That is one reason withdrawal alone is not treatment. If someone is very drowsy, breathing slowly or cannot be woken, call 999.
Does residential rehab replace methadone or buprenorphine?
Residential care and opioid substitution treatment answer different needs. WHO says opioid agonist maintenance treatment, with medicines such as methadone and buprenorphine, has the strongest evidence of effectiveness and cost-effectiveness for opioid dependence. Methadone maintenance has been offered in Malaysia since 2005, following Ministry of Health procedures.
WHO's clinical guidelines also say that withdrawal management on its own is unlikely to lead to lasting abstinence and is an important first step before psychosocial treatment, and that people who repeatedly relapse after withdrawal are likely to benefit from methadone maintenance or another substitution treatment. At Salus, the medical team reviews what the person takes as part of the assessment. We do not promise any medicine in advance, and nobody should stop a prescribed medicine on their own to come here.
How long does opioid rehab take, and what does it cost?
Most people need long-term treatment after detox, according to MedlinePlus, and NIDA's review of treatment research finds that most need at least three months, with the best results from longer stays. Salus is a 90-day stay for that reason. The four phases, Restore, Connect, Participate and Integrate, move on when the person is ready, and the aftercare plan is written well before the last week.
Salus charges a single fee of RM7,000 per month, so a full 90 days is about RM21,000. Round-the-clock medical oversight is inside that figure, together with the room and meals, therapy on your own and in a group, family sessions and the aftercare plan. Payment plans can be discussed.
Questions families ask.
What counts as an opioid?
Opioids include heroin and morphine, strong prescription painkillers, and codeine, which is found in some cough syrups. The drugs differ in strength, but dependence builds in a similar way and so does withdrawal: restlessness, muscle and bone pain, poor sleep, diarrhoea and vomiting. If the problem began with a painkiller or cough syrup, the same treatment applies.
My son refuses help. What can I do?
Keep the relationship, because people usually accept treatment from someone who stayed in their corner. Speak when he is calm, not high or in withdrawal, and describe what you have seen rather than listing his faults. Ultimatums sometimes work in the short term and usually cost trust. You can phone Salus without him knowing, and ask how family sessions work.
Does Salus give methadone or buprenorphine?
Medication decisions are made by the medical team after the assessment, and nothing is promised in advance. If someone is already prescribed methadone or buprenorphine, tell us exactly what and how much when you call, and do not stop it on your own. That is the most useful thing you can do before the assessment.
What happens after the 90 days?
Every guest leaves with a written aftercare plan, continued counselling and a support group, and continued support once home. Families are part of that plan. It includes knowing that tolerance is lower after time without opioids, so a return to an old dose is dangerous, and knowing who to call early if things start to slip.
Will other people find out he is at Salus?
Not unless you tell them. He stays as a guest of the hotel, and clinical information is held only by the care team. A first call to us is confidential and carries no obligation, so you can ask your questions before anyone decides anything.
